  • Must be a valid Citizen’s of the United States.
  • In the event of death of a Member who is 75 miles of more away from his or her legal residence at the time of death, APASI will render every assistance, and pay for locating a local, licensed funeral home, mortuary or direct disposition facility, arranging for the transportation of the body from the site of death to the licensed funeral home, mortuary or direct deposition facility in order for such licensed funeral home of facility to prepare the body for transport, purchasing the minimally necessary casket or air tray for transport, arranging and paying for the transportation of the remains to the licensed funeral home or mortuary within the United States, Canada, or Mexico and procuring the consular service (for death overseas) and death certificates (5). Important: Please note that traveling, airline regulations and/or foreign governments may require embalming or cremation for transportation to your receiving mortuary in the area of your legal residence.
  • Return of remains services are provide by APASI when Members are traveling more than 75 miles or more from their legal residence* or in another country of residence. All services must be arranged by APASI. NO CLAIMS FOR REIMBURSEMENT WILL BE ACCEPTED. The Plan of Assistance selected must be compatible with any pre-arranged agreement existing between the Member and the Enroller/Funeral home. This contract is non-negotible.
  • Legal residence is defined as the place of residence for 180 or more days in the twelve months preceding the date of death. It will require verification through voter registration, driver’s registration, and/or other means. A nursing home will be deemed the residence if the stay has exceeded 180 days.
  • If enrollment in APASI occurs while the Members is away from their legal address, plan of assistance becomes effective upon the Member returning to their legal address for subsequent travel. If the sales of the service is made away from the seller’s place of business a three day notice of cancellation is mandated. (at time of death) coverage cannot be guaranteed if member dies in a location cited on the United States State Department’s list for restricted travel by United States citizens.
  • Funeral Home & Pre-need Insurance Company information is not required on form.
